WILLIAMSTOWN -- Sweet Brook of Williamstown Re habilitation and Nursing Center will be hosting a "Health Care Proxy Day" on Thursday, Oct. 25, from noon to 4 p.m.
This is a free service open to the public.
A health care proxy is a legal document used in the event that you are unable to make an informed health care decision for yourself. You may authorize another person to act on your behalf regarding your health care related decisions and preferences.
Don’t wait until a crisis presents itself, be prepared. Don’t position your relatives to "guess" your intentions.
Staff at Sweet Brook will walk participants through the process clearly and carefully and if desired, Sweet Brook will provide your hospital of choice and/or primary care physician with a copy of your health care proxy form for your records.
This service will be provided in a private conference room adjacent to Sweet Brook’s reception lobby.
Light refreshments will be provided and packets of information will be made available for perusal at your leisure.
